Position Description

New! Full-time RN Supervisor!
Assess and monitor clients receiving services that enable them to remain safely and independently in their homes. Report to the Director of Home and Community Services. Supervise the job performance and care delivery Home Care Aides.

1. Home and Community Based Waiver Services / Case Management (70%)
- Perform RN assessment and coordination of care for home care clients using designated forms, software, and other health information to develop an individualized care plan.
- Work with the Home Care Scheduling Coordinator to oversee the staffing of client cases, review plans of care and aide duties. Facilitate client and Aide introductions.
- Review Home Care Aide documentation every 2 weeks to assess adherence to care plan and potential need for care adjustment.
- Perform client visits to include initial 30-day, 60-day, 6 months, and annual follow-up to assess each client for continued service eligibility based on program guidelines.
- Complete annual reassessments on all assigned clients to ensure plans of care and services rendered continue to meet client needs.
- Performs thorough review of clients’ records to ensure compliance with program requirements.
- Investigates suspicions of exploitation, abuse or neglect and report cases to the local law enforcement (APS/CPS).
- Make referrals or connect clients with other support systems to assist in their care.
- Ensure client orders for service are addressed and entered immediately into the scheduling software and that new client files are complete and accurate within 48 hours of opening a new case.
- Ensure that discharged client files are closed and complete within 2 weeks of the last date of service.
2. Supervisory responsibilities (20%)
- Provides support over the phone and in person to Home Care Aide staff.
- Supervise Home Care Aide staff: Provide counseling and disciplinary actions.
- Provide annual performance evaluations for Home Care Aide staff.
- Provide training to ensure Home Care Aides receive mandatory 12-hours of in-service annually.
- Provide training to Home Care Aides on usage and maintenance of specialized equipment based
on clients’ medical needs (e.g. Oxygen tank, Hoyer lift, catheters).
3. Quality Assurance (10%).
- Record management: Maintain records in compliance with program guidelines. Records must be
timely, accurate, complete, accessible, and in compliance with retention limits.
- Participate in the improvement of activities regarding quality of care and quality of record keeping.
- Active participant in the quarterly Home Care department meetings.

Handle daily activities independently. Plan weekly work in coordination with Home Care administrative staff and Home Care Aides. Refer to supervisor in case of conflicts with Home Care Aides, clients, and to report of suspicions of elder exploitation, abuse or neglect.

- Works well with other Home Care staff to ensure continuous care coverage for all clients across the service area of Bay Aging.

Has working knowledge of:
- Aging process and its impact on cognitive and physical abilities, geriatrics.
- HIPAA regulations.
- Microsoft Office programs (Word, Excel, Email).
- Regulations issued by the Office of Licensure and Certification, Department of Aging and Rehabilitation Services (DARS), the Department of Social Services and other state/federal agencies with regard to implications for care management and client services.
- Agency’s policies, procedures and operating standards.
- Meet continued competency criteria for RN licensure renewal requirement.
- Stay current in the knowledge of the profession through workshops, webinars, and trainings as assigned.

- Work in the office and various care locations efficiently and independently.
- Establish and maintain effective relationships with a variety of people (e.g. Home Care Aides, clients and their families, office co-workers, and external partners).
- Present ideas and concepts to a variety of small audiences (e.g. training of caregivers, new clients and their families) and adapt the message to the audience.
- Is flexible and able to prioritize activities as required by the circumstances.

Work is performed in many different home settings requiring the ability to lift up to 25lbs, walking and climbing stairs. Require regular independent travel around the area served by bay Aging.

10% of the essential functions of the position can be performed virtually away from usual work locations with remote access to software.

Must be licensed by the Virginia Board of Nursing or hold a current multi-state licensure privilege to practice nursing in Virginia as a Registered Nurse. Have at least 2 years of experience in the medical field or long term care environment.

Must possess a valid driver’s license and proof of PPD vaccination/screening.

FLSA status: This is an exempt position.
