Position Description

As a part of a multidisciplinary team, the NCQA Coordinator will manage the work to obtain and retain the NCQA credential for the organization, and may provide direct services including individual, therapeutic services to clients, such as assessments, care plans, and discharge planning.

Primary duties include:
• Perform functions necessary to obtain and maintain NCQA accreditation, including:
o Conducts a thorough evaluation of policies, protocols, and procedures to ensure NCQA requirements are met.
o Works with all departments to review, revise, and prepare policies, protocols, and procedures for NCQA credentials.
o Completes Gap analyses comparing standards to our organization’s processes.
o Provides guidance and direction for document preparation, review and finalization of documents for submission for NCQA credentials;
o Provides training to the team regarding NCQA process, requirements and steps to attain NCQA credentials designation.
o Works with organization to submit the online application to obtain the NCQA credentials designation and other reports as necessary.

Secondary duties may include:
• Addresses referrals from all sources.
• Participates and assesses daily needs of clients, coordinates services, and enters into database.
• Maintains weekly scheduled appointments in our electronic records systems.
• Provides therapeutic services to clients and their families.
• Works in collaboration with multidisciplinary team members including external partners.
• Completes required documentation and paperwork including assessments and care planning and implementation in a timely manner.
• Demonstrates cultural competency and commitment to patients.
• Demonstrates commitment to the philosophy of the agency.
• Maintain case load appropriate to current workload
• Other duties as assigned.

The incumbent in this position may have supervisory responsibility. He or she will be expected to provide leadership and instruction to colleagues and volunteers.

The environment for this position will be in the clients’ homes and in an office setting. In addition, the incumbent maybe required to travel via personal vehicle to client homes.

In the course of performing this work, the incumbent:

• Will spend substantial time standing, sitting, speaking and listening
• Must regularly lift and/or move up to 10 pounds, and occasionally lift and/or move up to 25 pounds

• Working knowledge of NCQA regulations and requirements, required; Knowledge and experience with preparing for and acquiring NCQA credentials for an organization, preferred.
• Exceedingly organized with strong attention to detail.
• Knowledge and experience working with the aging population.
• Strong clerical/administrative skills including personal computer skills (Microsoft Office software).
• Ability to make routine decisions and determine proper action based on experience within the limits of policy and procedures.
• Demonstrated oral/written skills.
• Knowledge of electronic medical records.
• Knowledge of community and community resources.
• Ability to work effectively with people of diverse cultures, ages and economic backgrounds a culturally competent and sensitive manner.
• Experience supervising staff.

• Licensed Clinical Social Worker or Registered Nurse in good standing in State of Virginia with minimum two years post-licensure experience.
• Degree in Health Care Administration, Business Administration, or similar field.
