Position Description

Bay Aging is seeking a Full-time Home Care Floater Aide-PCA for Middle Peninsula & Northern Neck, VA! Candidates will provide personal care, health and safety monitoring and light housekeeping. Introduce and coach new Personal Care Aides in the field. Perform administrative work in the office to complete a full-time schedule every week. Provide staff coverage at Bay Aging’s Adult Day Care Centers when needed. Report to the Registered Nurse (RN) Supervisor. Pay Rate: $19/hr-$20/hr

Essential functions

· Follow the individual Care Plan developed by the RN Supervisor.

1) Assist with Activities of Daily Living (ADL)

- Bathing, dressing and grooming

- Toileting

- Transferring

- Assist with eating

- Ambulation

- Turn and change position

2) Provide assistance with Special maintenance

- Check vitals,

- Supervise medication

- Assist with specialized care need as assigned by the RN supervisor

- Provide supervisory care for clients who require constant monitoring.

3) Perform Independent Activities of Daily Living (IADL)

- Meal preparation.

- Light housekeeping in the area of the home used by the client.

- Laundry.

- Accompany client to medical appointments, post-secondary school or in the community setting when assigned.

· Provide assistance with errands outside the home. Run errands within the local area for the client (e.g. grocery store, pharmacy, Post Office)

· Monitor home environment for health and safety.

· Communication and training.

- Document all services provided. Complete Aide records each day. Submit Aide records as directed.

- Communicate all changes in client’s physical or mental condition to RN Supervisor including suspicions of exploitation, abuse or neglect.

- Complete at least 12 hours of formal training annually.

- Provide on-the job training and coaching to new PCAs.

· Office work (up to 5%) – Work under the direction of the RN Supervisor or Scheduling Coordinator, to perform office administrative duties including but not limited to filing charts and making copies.

DECISION MAKING

Perform a limited number of routine tasks prescribed by clients’ care plan. Work independently throughout the day. Report to supervisor any change in the client’s environment, physical or mental condition.

KNOWLEDGE

Working knowledge of:

- HIPAA regulations.

- Adult care practice and procedures.

- Bay Aging policies and procedures related to Home Care.

Has knowledge of medical equipment on usage and basic maintenance such as Hoyer lift, gait belt, wheelchairs, oxygen tanks, catheters, glucometer and any specialized equipment required by the care plan. Stay current in the knowledge of their profession by completing at least 12 hours of in-service annually.

ABILITIES

- Speak, read and write English

- Establish and maintain good relationships with clients and their families even in difficult circumstances.

- Communicate effectively with older adults suffering from dementia or Alzheimer’s disease. Communicate with clients’ families to ensure good continuity of care.

- Be considerate of care recipients and hold their personal information as confidential.

- Be observant of changes in client’s condition or environment.

- Ability to stand, bend, stoop, pull, push wheelchairs and lift up to 25lbs unassisted.

WORKING ENVIRONMENT

The work is performed in the client’s home and in public areas. It requires repeated standing, bending, stooping, pulling, pushing and lifting of weights up to 25lbs unassisted. Regular night, weekend and holiday work required on a regular basis.None of the essential functions of the position can be performed virtually away from the assigned places of work.

QUALIFICATIONS

- Must be 18 years of age.

- Hold a Certificate of Completion for a Personal Care Aide (PCA) course that meets the Department of Medical Assistance approved curriculum

- Must provide proof of current PPD vaccination or complete a Tuberculosis screening.

- Become certified in CPR.
